QoS: fix NULL qos_block deref in qos_perexport_insert

qos_perexport_insert(export, NULL) is valid and means "use g_qos_config"
via lqos_block. The USE_MONITORING path still used qos_block->qos_type,
which SIGSEGV'd when EnableExportQosBwControl passed NULL.

Use lqos_block for the Per_Export metrics check.

Crash site: nfs_qos.c qos_perexport_insert(), qos_block == NULL.
