MDCACHE: Clean up refcounting and LRU

Streamline refcounting so all callers use mdcache_lru_ref and unref.

Promote from L2 to MRU of L1 (old code promoted to LRU of L1 which was
problematic).

Eliminate unused and confusing flags and return values.
