CEPH: Handle first time ACL setting

When nfs4_setfacl -s is used for the first time (set, not append), the
NFS client sends only the named user/group ACEs it was given - no
OWNER@, GROUP@, or EVERYONE@ ACEs. fsal_acl_2_posix_acl then leaves
ACL_USER_OBJ, ACL_GROUP_OBJ, and ACL_OTHER with zero permissions. Need
to fill in this info while setting the ACLs
