Set sbd_blocked_cookie correctly In "state_add_grant_cookie", the sbd_blocked_cookie field initialized in the begining. On failure of "do_lock_op", allocated cookie is freed, but sbd_blocked_cookie value kept as is, leading to double free condition. Also on freeing cookie, its entry from hash table should be removed.
